If you are a resident of CA, CO, CT, MT, NV, OR, TX, UT, VA or WA, or were in the last 12 months, you may exercise the data subject rights set forth below. To do so, please fill out the below Data Subject Request (DSR) form and describe your request. This form is directed to US residents who have certain rights as data subjects in relation to their personal data under some state or federal laws.

Requests

Covered consumers may make the following requests regarding the personal information that we may process:

“Right to Know Categories,” which is the right to know the categories of personal information we have collected of you, the categories of sources, the categories of personal information shared or sold, the categories of third parties with whom we have shared or sold the personal information, and the purposes for which personal information was collected or sold.  This right covers the 12-month period before the date we receive the request.
“Right to Know Specific Pieces,” which is the right to know the specific pieces of personal information that we have collected of you. This right covers the 12-month period before the date we receive the request.
“Right to Delete” the personal information retained by us, subject to some exceptions that would permit us to keep the information.
"Right to Correct" any inaccurate personal information that we maintain about you.  
“Right of Portability” the right to have your information transferred to you so that you can transfer it so another entity.

How we verify your identity

In order to verify your identity, we will ask, at a minimum, that you provide your name, email address, and mailing address so that we can match this information with the information existing in our systems

First, we will send you a verification email to verify that you have access to the email that you provided in Section 1 above. Then, we will then use the information you provided above to verify your identify to a reasonable degree or a reasonably high degree of certainty, depending on the request. If we are able to match at least two pieces of personal information to the personal information that we may retain of you, we will honor your Request to Know Categories. If we are able to match at least three pieces of personal information to the personal information that we may retain of you, we will honor your Request to Know Specific Pieces, your Request to Delete, and your Request to Correct. If we are unable to verify your identity, we will let you know that we are unable to fulfill your request.

Timeframe

We will respond to and process your request within 45 days and if you are located outside the United States, within 30 days. However, if we need more time to respond, we may take an additional 45 days (90 days in total). You will be notified if we need an additional 45 days.
